Your i-693 Process with San Diego Docs


For those who have enrolled with San Diego Docs, we are here to help you complete your immigration evaluation (i693) for USCIS. We’ve outlined the steps below. If you have any questions, please feel free to contact us.


1. Please email us any vaccination records that you have available to info@sandiegodocs.com

with the subject line “IE Vaccination Records - (Your Name)”.


2. Your blood work has been ordered to Quest Diagnostics. You can go to any of their locations to have the blood work drawn. It is often faster to schedule an appointment with them at

https://appointment.questdiagnostics.com/as-home. If you chose to have the mobile phlebotomist come to you, then they will contact you to setup the most convenient time

and location.


3. If you have any necessary vaccinations, they have already been ordered to:

4. Once your labs have resulted and you’ve received the vaccines, you will then be scheduled for your in person appointment for exam and completion of remaining paperwork with one of our providers. This will be at either:

Please bring a government issued picture ID and copies of your vaccination records to the appointment.


You will be given the completed i693 form in a sealed envelope as well as a copy for your records.

i693-Report of Immigration Medical Examination and Vaccination Record Frequently Asked Questions

  • What is the purpose of the I-693 medical exam?

    The I-693 exam is a USCIS requirement for all people immigrating to the United States to determine if you have any health conditions that could:

     ◦ Be a danger to public health

     ◦ Impair your ability to work or attend school

     ◦ Require extensive medical care after your arrival in the U.S.

  • Who needs to complete the I-693?

    Most individuals applying for an immigrant visa or seeking to adjust their status to a permanent resident must complete the form. 


    There are some exceptions, so always consult the USCIS website.

  • What does the medical exam involve?

    The exam includes:

     ◦ Review of your medical history

     ◦ Physical examination

     ◦ Blood tests (for syphilis, gonorrhea, tuberculosis)

     ◦ Vaccinations (if needed, according to your age and medical history)

  • How much does the I-693 exam cost?

    The costs for the medical examination, lab tests, and completion of all necessary forms is $425. Any required vaccinations are a separate fee. 

  • What documents do I need to bring to the appointment?

     • Government-issued photo identification (passport, driver's license)

     • Vaccination records

     • The I-693 form (partially completed in advance)

     • Any documentation of prior medical conditions

  • How long is the I-693 form valid?

    Effective April 4, 2024, any Form I-693 that was properly completed and signed by a civil surgeon on or after Nov. 1, 2023, does not expire, and can be used indefinitely as evidence to show that the applicant is not inadmissible on health-related grounds.

  • What happens after I-693 is completed?

    After you complete the medical exam and all the required paperwork is signed, the civil surgeon will hand you a sealed envelope with your results inside. 


    This special envelope can not be opened by you and needs to be sent directly to USCIS along with your immigration application.

  • What happens at the medical exam for I 693?

    Your blood work and vaccinations should already be completed before the brief physical examination.


    They will discuss your past medical history and complete the form while you are there. 


    You will need to sign the form in the presence of the examiner (do not sign the form beforehand).


    The original signed document will be given to you in a sealed envelope (do not open this envelope) to be delivered to USCIS and a copy will also be provided to you or your records.

  • Can I submit the I-693 myself, or does the doctor do it?

    The Civil Surgeon will give you the completed form in a sealed envelope. 


    Do not open the envelope. 


    You must submit it to USCIS as instructed with your immigration application.
